Our primary mission is to partner with you in creating a healthy life, where you can flourish! Services are offered both one to one and in group settings, and sessions can be provided via in-person visits and/or Telehealth visits! Since we are community-based, we specialize in meeting your needs with a tailored approach to flourishing within your home community! 

We welcome people of all ages and abilities, with specialty treatment areas including developmental & intellectual disabilities, orthopedic disabilities, neurological disorders, addictions & recovery, and chronic health conditions. As chronic health conditions are notably on the rise, we also serve those with heart disease, stroke, diabetes, obesity, and a sedentary lifestyle.
